Extended
Dental Plan
Extended Dental is used to be combined
with your primary dental coverage and to waive your copayment up
to 20% . Employees or employers have the option to use this benefit
to eliminate the copayment of up to 20% of Basic dental services
through participating dentists.
The plan match the limit of the
primary dental insurance coverage and pays up to 20 percent of treatment
cost based on primary insurance fee coverage.
Example;
" If you have
dental insurance through your work that covers for basic dental
treatments like fillings, tooth extrication or cleaning at 80% with
$2500 annual limit, you may choose to become Extended Dental Member
for as low as $10 per month and increase the primary coverage to
100% up to your $2500 limit, a saving of $500 a year in this example."
Extended Dental can be purchased
by employer as an adjunctive to primary plan OR to be purchased
by individual employees. It is not offered to Individual dental
plans and can not be combined with any other memberships with Benefi
Dental membership plans.
